What is Community Stabilization
A short-term behavioral health service designed to prevent hospitalization and support individuals experiencing a mental health crisis. This service provides intensive support to individuals experiencing a psychiatric crisis. Services are delivered in the home and community to reduce hospitalization, stabilize and restore functioning.
Who Qualifies?
Must be:
- Medicaid Eligible
- Experiencing acute mental health crisis.
- At risk of psychiatric hospitalization.
- Recently discharged from a higher level of care
(23-Hour Crisis Stabilization, Acute Psychiatric Inpatient Services, ASAM levels 3.1 – 4.0, CSB Emergency Services, Hospital Emergency Department, Mobile Crisis Response, Partial Hospitalization Program (Mental Health or ARTS), Psychiatric Residential Treatment Facility (Mental Health or ARTS), Residential Crisis Stabilization Unit, Short-term detention or incarceration, Therapeutic Group Home
Services Include
- Crisis Intervention
- Psychiatric Stabilization
- Medication Monitoring/Support
- Safety Planning
- Care Coordination/Linkage to ongoing outpatient services
- Family Support
Length of Service
Services are typically provided up to 7 to 10 days and may be extended based on clinical necessity.
